That relationship is established only when you are formally accepted as a patient and complete our intake process. 3. Emergencies
If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use this website, our contact forms, or text messaging to report an emergency. These channels are not monitored continuously and are not an appropriate way to seek urgent help.

You must be at least 18 years old to use this website, submit information through it, or enroll in our text messaging program. If you are under 18, a parent or legal guardian must do so on your behalf. By using the site, you represent that you meet this requirement.
5. Your Responsibilities

We will provide the services you engage us for with the professional care and skill required of a healthcare provider, in accordance with applicable law and professional standards. We will handle the information you give us in accordance with our Privacy Policy and, where it applies, our Notice of Privacy Practices under HIPAA. We will maintain this website and make reasonable efforts to keep its information accurate, and we will honor your communication preferences, including any request to stop receiving text messages.
We do not guarantee any particular medical outcome. Treatment decisions are made by your treating provider based on your individual circumstances.
7. Text Messaging Terms
Our text messaging program operates under the program name ARC Medical Patient Messaging. By opting in, you agree to the terms in this section.
Consent
We send text messages only to individuals who have given express consent to receive them, by providing a mobile number and agreeing to receive text messages on one of our forms, or by requesting text communication from our staff. Consent to receive text messages is not a condition of receiving treatment from us or of purchasing any good or service.
What we send
- Appointment confirmations, reminders, and scheduling or rescheduling notices;
- Care coordination and clinical follow-up, such as referral and imaging status, therapy follow-up, and responses to questions you send us;
- Account and administrative notifications, such as records requests and billing notices;
- Occasional requests for your feedback or a review of your experience with our clinic.
Frequency, cost, and delivery
- Message frequency varies per user.

- Text HELP for help. Text STOP to unsubscribe.
- Carriers are not liable for delayed or undelivered messages.
Help and opting out
Reply HELP to any message to receive our contact information and assistance with the program.
Reply STOP, END, CANCEL, UNSUBSCRIBE, or QUIT to any message to stop receiving texts. You will receive one final message confirming that you have been unsubscribed, after which we will send no further texts unless you opt in again. You may also opt out by calling (801) 697-6006 or emailing [email protected]. If you have opted out and wish to resume, reply START or contact us.
Opting out of text messages does not affect your care. We will continue to reach you by phone, mail, or email as needed.
Security of text messages
Text messaging is not a secure or encrypted method of communication. Messages may contain information about your care and can be viewed by anyone with access to your device or mobile account. By opting in, you accept this risk. You may ask us at any time to communicate with you by alternative means or at an alternative location, and we will accommodate reasonable requests.
Carriers and devices
Message delivery depends on your mobile carrier and device and is not guaranteed. Wireless carriers are not liable for delayed or undelivered messages. Not all mobile devices or carriers may support our messages.
8. Appointments and Cancellations
Submitting a request through this website is a request, not a confirmed appointment. An appointment is scheduled only when our staff confirms it with you directly. If you need to cancel or reschedule, please give us as much notice as you reasonably can so we can offer the time to another patient. Specific scheduling, cancellation, and missed-appointment policies are provided to you at intake.
9. Billing and Payment
Fees, insurance, and any third-party payment arrangements relating to your care are governed by the financial agreement and related documents you sign at intake, not by these Terms. Please contact our office with any billing questions.
10. Intellectual Property
The content on this website — including text, graphics, logos, images, and the ARC Medical name and marks — is owned by ARC Medical or its licensors and is protected by copyright, trademark, and other laws. You may view and print pages for your own personal, non-commercial use. You may not otherwise copy, reproduce, republish, distribute, or create derivative works from this content without our prior written permission.
11. Links to Other Websites
This website may link to sites we do not control, such as a patient portal or scheduling tool. No waiver. Our failure to enforce any provision of these Terms is not a waiver of our right to enforce it later.
